Open Positions

Oversees the daily operations of our clinical team ensuring compassionate, high-quality care that meets established standards. Collaborates with the interdisciplinary team to coordinate patient care, provide strong leadership and mentorship and support staff through recruitment, training, and evaluations. Requires an active Hawaiʻi RN license, at least two years of hospice and palliative care experience, strong leadership and communication skills, prior healthcare management experience, and reliable transportation with a valid driver’s license and auto insurance.

Provides direct services and support to hospice and palliative care patients and their families by conducting psychosocial assessments, assisting with care planning and documentation, and working closely with our team to ensure holistic, high-quality care. Requires a master’s degree in social work (MSW) with a current and active license to practice social work in the state of Hawai’i; experience in hospice, palliative care, or a related healthcare field; in-depth knowledge of CMS guidelines for hospice care; strong communication and interpersonal skills; compassionate and empathetic approach to patient and family care; and reliable transportation with a valid driver’s license and auto insurance. LCSW candidates are strongly encouraged to apply.

Delivers compassionate, hands-on care to patients under the supervision of a Registered Nurse, helping with daily activities such as bathing, dressing, grooming, toileting, mobility, and light homemaking tasks. The role also includes documenting care, observing and reporting changes in patient conditions, and ensuring patient safety. Qualifications include the ability to read and write in English, a current state Certified Nursing Assistant (CNA) certification, completion of required federal and state hospice aide training and competencies, at least one year of recent experience as a CNA without a 24-month lapse in service, knowledge of home care and the dying process, effective communication skills, and a valid driver’s license with proof of auto insurance
